If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
- Driver’s License or State ID
- Referral (If Required by Insurer)
- Insurance Information
- Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
We accept the following insurance providers:
- Blue Cross
- United Healthcare
- Web TPA
- PPO Plus
- Workers Comp.
We ask that you make your appointments in advance so we ensure everyone is seen in a timely manner. In the event you are unable to make your appointment, please call us so that we are able to fill your spot with a person in need of our care. Fees & Payments
We accept cash, checks, MasterCard, and Visa for your convenience. We will bill accepted insurance companies directly on your behalf. Patients should be aware that is their responsibility to pay all necessary fees incurred. Our staff is here to assist you in handling claims any way that we can.
